Health Center Quality Improvement 2020 Grant Award Recipient! Tri-County is pleased to announce that we have been recognized by the Health Resources and Services Administration (HRSA) as a health center that has “made significant quality improvement gains from the previous year.” Tri-County Health Centers were recognized as overall Quality Leaders, gold status, and were also recognized as having made significant improvements in Health Information Technology, access, and reducing health disparities. Tri-County offers comprehensive family medicine, integrated behavioral health, and dental health services, at two convenient locations. Our staff of highly trained medical professionals include physicians, nurse practitioners, licensed clinical social workers, dentists, dental hygienists, nurses, medical assistants and medical receptionists, along with other skilled health care professionals. We offer a patient-centered approach to care, and follow evidence-based guidelines according to the needs of our patients, including the American Academy of Pediatrics for well-child visits, and the American Academy of Family Physicians for adult well visits.
Our state-of-the-art health centers are equipped to provide care for patients of any age, from prenatal and infant care, to pediatrics and adolescents, to adult primary care and senior care. We offer convenient hours, evening appointments and same day visits, at both the Johnston and North Providence locations.
Tri-County does not refuse care based on an inability to pay. We offer sliding scale discounts to those that qualify based on family size and income. Most health insurance plans are accepted. Individuals of all ages are welcome at Tri-County. We do not discriminate on the basis of race, color, religion, national origin, age, handicap (disability), veteran status, pregnancy, marital status, genetic information, medical condition, sexual orientation, gender identity, limited English proficiency (LEP) or any other status protected by Federal and state law.
Tri-County Health Centers receive Health and Human Services funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
