Tri-County’s Personal Choice Waiver Program services individuals with disabilities aged 18 years and older, and elderly men and women aged 65 years and older, who are financially qualified for Medicaid. The program provides long-term home based and community based services and resources with a focus in keeping individuals in their homes, communities or assisted living residences, and out of nursing homes. Additionally, the Personal Choice Waiver Program allows individuals the choice of their care provider.
Qualified clients may represent themselves or have a representative (often a family member) who assists them with hiring care staff and overseeing the care staff with time tracking. The Program goal is to reduce unnecessary institutionalization by providing specialized long-term home and community base services to eligible participants.

The client must be 18 years of age or older (if disabled) or 65 years or older. Clients must be Medicaid eligible and meet a high or highest level of care services according to the Centers for Medicare and Medicaid.
